The form is segmented into each Generation Line. Each Sire/Dam location number (e.g. Sire – 1) corresponds to the Red Numbers on the Example Pedigree Chart

For Dam or Sire: enter horse’s name. For Pedigree: enter all known information (Breed, Registry, Registration #). Enter “unknown” if information is not known.
